Filling Out Form

Enrollment Process

01

Referral/ Intake documents

First, we receive a referral from an insurance provider. This is only applicable for those using insurance- if you're paying out of pocket then this step will be skipped. Next, we ask that you fill out the intake packet in order to get some basic background information for services.

02

Assessment/ outcome measures

After we receive the intake documents, we then schedule an initial assessment to gather more information and make our first initial observations. During this assessment, you expect the provider to directly observe the child/client, gather background information, conduct a functional behavior assessment, talk about the results from parent/caregiver interview and reporting scales, and other related questions towards developing a treatment plan.

03

Outcome Measures and Authorization

Next, outcome measure scores will be evaluated prior to treatment authorization in order to develop a detailed plan and to evaluate the client's progress or regression over time. The type of measurement will depend on the insurance provider. Tricare specifically has age-based outcome measures like PDDBI, the Vineland Adaptive Behavior Scale, and other measures. Afterward, your insurance provider will approve or decline the treatment based on the results of our outcome measures and assessments.
